METHOD OF FORMING AN EXTERNAL GASTRIC FISTULA FOR NUTRITION IN PATIENTS WITH A THIN ANTERIOR ABDOMINAL WALL Russian patent published in 2021 - IPC A61B17/00 A61M25/10

Abstract RU 2745655 C1

FIELD: medicine.

SUBSTANCE: invention relates to medicine, namely to surgery. Before performing the operation, the tip of the Foley catheter with side holes is cut off while preserving the balloon. An incision of the anterior abdominal wall is planned in the projection of the upper third of the stomach body, focusing on the location of the gas bubble of the stomach according to the X-ray of the abdominal organs before the operation, which corresponds to the fundus of the stomach. An incision of 3 cm is made, sufficient for the insertion of the early-expanding hooks and the gastrostomy tube. At 1.5 cm from the edges of the future incision of the stomach wall, stitches are applied, of which 2 stitches are oriented along the axis of the wound of the anterior abdominal wall. A Foley catheter with an inflatable balloon at the end is inserted into the stomach lumen, the catheter balloon in the stomach lumen is filled with water. The Foley catheter is pulled out through the wound of the anterior abdominal wall until the stomach wall and the anterior abdominal wall are pressed. The stomach wall is sutured to the parietal peritoneum of the anterior abdominal wall with the free ends of the retainer sutures, and the edges of the sheath walls of the rectus abdominis muscle are captured in the upper and lower sutures along the wound.

EFFECT: method does not require general anesthesia, reduces the risk of inflammatory complications from the wound, promotes early activation of the patient, reduces the risk of thromboembolic complications and hypostatic pneumonia.
