METHOD FOR FIXING DRAINAGE TO WOUND AFTER SINGLE-PORT TORACOSCOPY Russian patent published in 2021 - IPC A61B17/00

Abstract RU 2747359 C2

FIELD: medicine.

SUBSTANCE: invention relates to medicine, namely to oncology and thoracic surgery. After the completion of the surgical intervention on the organs of the chest, drainage is placed into the pleural cavity. Drainage is installed in the corner closest to the operator along the posterior surface of the pleural cavity into the dome. A 20-22Fr drain and 2-0 Vicryl thread are used. It’s necessary to begin to suture the access with a continuous suture from the corner of the wound, where the drainage is located. Moreover, the muscles are sutured separately in the first row, and in the opposite direction - the subcutaneous fat. The thread is tied, the ends are cut off. Next, a separate U-shaped seam is applied around the drainage, the thread is tied to one knot. After that the drainage in the wound is fixed in a twisted way. After that, the skin is sutured with a continuous Halstead suture with atraumatic Vikryl 3-0 suture from the opposite corner where the drainage is located. At the drainage, the thread is tightened and a loop is formed. Then, with the same thread, provisional sutures are applied around the drainage and the needle is punctured through the skin 1-2 cm from the corner of the wound. The thread is fixed to the skin. After removing the drainage from the pleural cavity, the U-shaped suture is tied. The skin edges of the wound are aligned by tightening the provisional sutures.

EFFECT: method makes it possible to create an effective tightness of the wound around the drainage, to reduce the risk of developing a wound infection in the area of the drainage setting, as well as to ensure the proper cosmetic effect of the wound after its healing.
