FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to traumatology and orthopedics, and can be used for preoperative planning of fixation of the acetabular component with screws, followed by arthroplasty of the hip joint. A polypositional fluoroscopy is performed and the supra-acetabular region is divided into three conditional equal parts, and in the center of each part, the points of introduction of cannulated screws are designated, forming a conditional triangle. The cannulated screws are installed so that only the threaded part of the cannulated screws can enter the bone tissue. MSCT of the hip joint is performed with cannulated screws installed as landmarks and a 3D model of the pelvis is designed. Based on the data obtained, a 3D model of an individual acetabular component is designed, on which the location of the center of rotation, angles of frontal inclination and anteversion, the number of holes, screws, their location, length, and direction are set. Next, a navigation flange with three holes is designed, corresponding to the arrangement of cannulated screws with the configuration of the inner surface identical to the outer surface of the bone in the supra-acetabular region. An individual monoblock implant is made. The defect of the acetabulum is verified, the supra-acetabular region with cannulated screws is isolated, and an individual monoblock implant is installed.
EFFECT: method provides a reduction in the time of preoperative planning and the operation itself due to the absence of the need to manufacture and use plastic models of a pelvic defect and implant templates, minimizes the risk of bacterial contamination of the surgical wound by eliminating the selection of implant templates directly in the wound during the operation, ensures high accuracy of preoperative planning, since the landmarks for fixing the acetabular component are set in the supra-acetabular region of the acetabulum and their position relative to the acetabulum is static, and the changes in the spatial position of the pelvic tilt for the acetabulum and the installed screws are identical, it allows identical installation and alignment of an individual monoblock implant due to installed cannulated screws as landmarks in the supra-acetabular region of the acetabulum.
