FIELD: medicine, jaw reconstruction in particular.
SUBSTANCE: group of inventions relates to medicine, namely to methods and devices for the reconstruction of defects of the upper jaw. The method for reconstruction of defects of the upper jaw includes preoperative planning of the volume of resection, modeling of the template according to the anthropometric data of a particular patient, designing the lower ocular wall in the form of a mesh plate and the selection of fasteners. The autograft is fixed to the mesh plate with fasteners from the lateral side to the zygomatic bone in different directions. Using a template, the autograft is modeled in exact accordance with the resected fragment of the upper jaw, the lower orbital wall is located along the upper edge of the autograft with fixation directly to the autograft, the zygomatic and nasal bones, in addition, the scapular autograft is fixed in the lower part in the area of the angle to the sawdust of the alveolar edge of the resected upper jaw using a titanium mini-plate. The device for the reconstruction of defects in the upper jaw includes a mesh plate, and the mesh plate is made using the technology of layer-by-layer fusion of fine metal powder, DMLS made from Ti64ELI material, the mesh structure of the plate is made with rhombic holes. The plate has lateral branches with a thickness of 4 mm to 6 mm with holes for fastening, which are located at a distance of 3.95 mm to 5.16 mm, and at the same time there is a branch with a bend on the medial side of the plate that follows the contour of the nasal bone.
EFFECT: increase in the efficiency of restoring the integrity of the lower orbital area of the zygomatic bone is achieved during the reconstruction of defects in the upper jaw by simplifying the fixation of the graft.
