SUBSTANCE: group of inventions relates to medicine, namely to oncology, maxillofacial and reconstructive plastic surgery. Planning of the resection of a bone tissue site and preparation of the lower jaw for reconstruction by resection and sawdust are performed The template is modeled according to the anthropometric data of a particular patient. Graft and template are designed. A J-shaped graft bed is prepared. The peroneal autograft is taken from the supplying vessels, which are the peroneal artery and vein. Modeling of the autograft is carried out according to the preliminary template. The peroneal autograft is fixed on the supplying vessels to the device for the reconstruction of the lower jaw. Then the device with the autograft is fixed to the sawdust of the resected lower jaw. The imposition of microanastomoses between the feeding vessels of the autograft and the facial artery and vein is performed. The method is implemented by means of a device for the reconstruction of defects in the lower jaw. The device has a mesh structure with holes made using the technology of layer-by-layer fusion of finely dispersed metal powder - DMLS from Ti64ELI material. The device consists of a mesh plate, repeating the contours of the defect with thickness of 0.9 to 1.2 mm. The inner side of the plate is made with the possibility of fixing the peroneal autograft to it. The sides of the device in the form of plates up to 0.5 mm thick and up to 1 mm high are located at the jaw-autograft border.
EFFECT: group of inventions makes it possible to reduce traumatism during the operation of reconstruction of the lower jaw and to increase the aesthetic effect.
