FIELD: medicine.
SUBSTANCE: inventions group relates to medicine, namely to ophthalmology. According to the first variant, the fibrovascular pannus is excised from the surface of the cornea of the affected eye. A limbal flap in the upper sector is cut out on the paired contralateral eye. Its fragments are transplanted onto the operated eye, a protective coating is applied. Fibrovascular pannus is excised in the eye with limbal insufficiency syndrome (LIS). Superficial symmetric radial incisions are made in the region of the corneal stroma periphery to a depth of 50 mcm. Tunnels 1 mm long are formed from each incision, the cut out limbal flap is divided into fragments and one fragment is placed in each tunnel. An amniotic membrane is applied as a protective coating on the entire surface of the cornea and fixed with sutures outside the limbus. In case of LIS, after excision of the fibrovascular pannus, the surface of the cornea is polished and the residual tissue of the fibrovascular pannus is removed no deeper than the Bowman layer with an ophthalmic microbore with a nozzle in the form of a ball with diamond dusting with a diameter of 0.5-5.0 mm in circular repetitive movements with equal pressure force, at an average speed of 10,000 rpm and continuous irrigation with balanced brine. According to the second option, de-epithelization and removal of calcium deposits from the surface of the cornea are performed mechanically. When the cornea calcifies, the surface of the cornea is polished no deeper than the Bowman layer with an ophthalmic microbore with a nozzle in the form of a diamond-coated ball with a diameter of 0.5-5.0 mm in circular repetitive movements with equal pressure force, at an average speed of 10,000 rpm and continuous irrigation with a balanced saline solution.
EFFECT: invention allows reducing the risk of intraoperative complications, due to uniform and less traumatic resurfacing of the corneal surface for the subsequent favorable growth of corneal epithelial cells, and, consequently, increasing transparency of the cornea.
