FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to ophthalmology. To reconstruct the corneal epithelium in unilateral limbal insufficiency syndrome (LIS), the fibrovascular pannus is excised from the surface of the cornea of the affected eye, the limbus fragments are transplanted into tunnels located on the periphery of the cornea, the amniotic membrane is applied and fixed, followed by the installation of a soft contact lens. Transplantation of limbus fragments is carried out simultaneously with femtolaser-assisted anterior layered keratoplasty, for this, on the recipient's cornea, after excision of the fibrovascular pannus, superficial keratectomy and detachment of the superficial layers of the stroma from the deep layers are performed, the cornea is excised, then the deep layers of the corneal stroma are exfoliated and removed. Form 8-10 equidistant from each other tunnels located on the periphery of the corneoscleral disk or on the periphery of the donor cornea. The donor cornea cut with a femtosecond laser is placed in the prepared operating bed, and fixing sutures are applied. Next, a rectangular limbus flap is cut out of the recipient's eye at 12 o'clock and divided into fragments, the number of which corresponds to the number of corneal tunnels. One fragment of the limbus is placed in each of the formed tunnels, an amniotic membrane is applied to the entire surface of the cornea, and it is fixed with a continuous absorbable suture.
EFFECT: method reduces the number of intra- and postoperative complications of transplantation and anterior corneal layer-by-layer transplantation, and also ensures stable graft engraftment, uniform and unimpeded growth of epithelial cells over the entire surface of the cornea, and an increase in visual functions in patients with LIS.
