METHOD FOR MINI-INVASIVE TREATMENT OF BILE DUCT DAMAGE Russian patent published in 2021 - IPC A61B17/94 A61B17/00 A61M25/01 A61M25/06

Abstract RU 2753386 C1

FIELD: medicine.

SUBSTANCE: invention relates to medicine, namely to abdominal surgery, interventional radiology, endoscopy. Percutaneous transhepatic cholangiostomy is performed. Endoscopic retrograde cholangiography is then performed with cannulation of the common bile duct with an endoscopic guidewire. The endoscopic guidewire is passed through the area of the duct damage into the biloma and through the bile fistula to the outside, simultaneously performing percutaneous transhepatic passage of an angiographic guidewire through the area of the duct damage into the biloma and through the biliary fistula to the outside. A catheter with a length of at least 45 cm is inserted through the angiographic guidewire so that the distal end thereof exits through the biliary fistula to the outside. The angiographic guidewire is removed, an endoscopic guidewire is inserted into the distal end of the catheter until the latter exits from the proximal end of the catheter, the catheter and the endoscopic guidewire are tightened until straightening in the area of the duct damage, while recanalisation of the area of damage is performed. External-internal biliary drainage is performed.

EFFECT: provides a possibility of performing a mini-invasive restoration of the bile passage into the intestine with high efficiency and minimal time expenditure, contributing to the closure of the biliary fistula, does not require the use of endoscopic loops.
