FIELD: medicine.
SUBSTANCE: invention relates to the field of medicine, in particular to abdominal surgery, surgical hepatology and can be used in treatment of oncological and other liver neoplasms that are localized in the VII and VIII segments of liver. To do this, abdominal access is performed. An optical laparoport is installed in periumbilical part. Diagnostic laparoscopy is performed. Additionally, three laparoports are installed: the second laparoport along the right mid-clavicular line, the third laparoport along the left mid-clavicular line and the fourth laparoport under the metasternum. For an overview thoracoscopy, a thoracoport is installed in the sixth intercostal space on the right along the middle axillary line. Also, thoracoports are installed for working trocars: in the fifth intercostal space along the anterior axillary line and in the seventh intercostal space along the posterior axillary line. After that, dissection of the right pulmonary ligament is performed and ultrasound examination is repeated, determining the boundaries of liver resection. To perform liver resection, the diaphragm is dissected in vascular-free zone in transverse direction with fixation of the upper edge of diaphragmatic flap to anterior chest wall by endoclips and separation of hepatic parenchyma with clipping of vessels and bile ducts. At the end of resection, drains are brought to resection zone from abdominal cavity, drainage is installed into the right pleural sinus, and defect of diaphragm is sutured.
EFFECT: method makes it possible to shorten the operation time, reduce blood loss and trauma, to improve operation results, as well as to shorten time of postoperative rehabilitation of patients by optimizing parameters of thoracoscopic and laparoscopic ports location in combination with a strictly specified procedure for performing individual techniques during the operation, which causes ergonomics of performing surgical manipulations in abdominal area, exclusion of tool conflict, high quality visualization of surgical field.
