FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to neurosurgery. All stages of the operation are performed through extraorbital trans-eyebrow access. A linear skin incision is made along the eyebrow line in its outer part. Next, the subcutaneous fat and periosteum of the frontal bone are dissected in a horizontal plane passing through the upper edge of the orbit. The underlying frontal bone is skeletonized to the upper edge of the orbit and the zygomatic process of the frontal bone below the level of the zygomatic suture. Next, the anterior portions of the temporal muscle with the fascia are incised within 1 cm posterior to the level of the anterior sections of the superior temporal line while maintaining the muscle cuff along the superior temporal line. The anterior sections of the temporal muscle are displaced to the base and the underlying temporal bone is skeletonized. The outer surface of the orbital edge is cut. Then, under endoscopic control, the lateral wall of the orbit is skeletonized to the level of the inferior orbital fissure. A hole is formed in the middle part of the lateral wall of the orbit. Next, the opening is expanded: downwards - to the lower orbital fissure with the involvement of the latter, upward - to the upper orbital fissure and the lacrimal fossa of the roof of the orbit, inwardly - to the orbital surface of the main bone with resection of the latter. The operation is completed by dissecting the periorbitis.
EFFECT: method allows reducing traumatism of surgical intervention, achieving pronounced decompression of the orbit, eliminating compression of the optic nerve, reducing exophthalmos and keratopathy phenomena, improving visual acuity.
