FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to ophthalmology. Non-penetrating deep sclerectomy and trabeculotomy with formation of conjunctival incision and superficial scleral flap are performed. Surface scleral flap is separated by 1/3 of the thickness of the sclera of a trapezoid shape with a limbal portion of 8 mm long, a scleral portion of 6 mm and width of 5 mm. A trapezoidal flap is cut out from the middle layers of the sclera with its base to the limb until the trabecula and the peripheral part of the Descemet's membrane are exposed by 1-2 mm. Then, three sclera strips are formed from the mid scleral flap by means of two incisions parallel to its edges and perpendicular to its base, the width of the outermost strips of sclera is 1.5 mm, the width of the central strip in the proximal part to limb is 5 mm, in the distal part – 3 mm. Further, at 2 mm from the limb along the scleral bed, a through parallel limb incision is made to ciliary body 7 mm long, from which anterior cyclodialysis is performed. Extreme strips of the sclera are successively implanted into the angle of the anterior chamber; a through incision of trabecula 2-2.5 mm long is performed at the base of the central strip, through which a basal iridectomy is performed. Through the incision of the deep layers of the sclera in the bed area and after the preliminary posterior cyclodialysis, the biomaterial "Alloplant" is implanted into the suprachoroidal space for the optic nerve revascularization with a limb width of 6 mm and length of 5 mm, and then on top of it remaining central strip of middle layers of sclera.
EFFECT: method enables achieving a stable hypotensive effect and improving the state of visual functions in the patients with initial, advanced and advanced primary open-angle, closed-angle and mixed glaucoma.
