FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely ophthalmology, and can be used in conducting anesthetic aids for laser vision correction in childhood. To do this, combined general anesthesia is performed using a laryngeal mask. Induction into anesthesia is carried out by a combination of inhalation of sevoflurane with a bolus load of 6 vol. %, followed by the administration of propofol at a dose of 3 mg/kg of weight intravenously (IV). Analgesia is achieved by the introduction of fentanyl at a dose of 2 mcg/kg of weight IV and the use of a local anesthetic, oxybuprocaine hydrochloride, which is instilled into the conjunctival sac, 1 drop before and after surgery. In premedication, for better recognition of the pupil with a laser, atropine is used at a dose of 0.01 mg/kg of weight, IV. Artificial ventilation of the lungs, controlled in Volume Control mode (respiratory volume: 6 ml/kg), is carried out with the possibility of triggering inspiration. Anesthesia is maintained by inhalation anesthesia with sevorane at the rate of 1.2 MAC, with a minimum alveolar concentration. To achieve the optimal position of the eye, the muscle relaxant atracuria bezilate is used at a dose of 0.1 mg/kg of weight without depression of the respiratory muscles.
EFFECT: method provides adequate anesthesia during laser vision correction in this category of patients, which causes the creation of an even eye position in the central area of the cornea under general anesthesia without manual control by the surgeon, while significantly reducing the likelihood of postoperative complications in the form of postoperative astigmatism.
