SUBSTANCE: invention relates to medicine, namely to neurosurgery. Median suboccipital access to tumors of the posterior cranial fossa is performed by incision of the soft tissues of the cervical-occipital region, trepanation of the occipital bone. In this case, a linear vertical incision of the soft tissues of the cervical-occipital region is carried out with a length of 4 cm from the spinous process of the second cervical vertebra in the direction of the external occipital protrusion. The retraction of the soft tissue array is carried out using minimally invasive wound expanders. Subperiosteal dissection of the scales of the occipital bone and the posterior arch of the first cervical vertebra is performed. Trepanation of the occipital bone is performed at a distance of 1.5 cm from the edge of the great foramen. The dura mater is opened with a semi-oval incision. In this case, the access path is chosen so that the line of sight under the microscope is located along the plane of the diamond-shaped fossa.

EFFECT: method makes it possible to improve the results of treatment by reducing the number of postoperative complications, the volume of blood loss, the time of application of analgesic drugs and the duration of the patient’s stay in the hospital.
