SUBSTANCE: invention relates to medicine, namely to operative oncology. Subcutaneous mastectomy is performed, the formation of a skin-fat pocket for the installation of a breast implant in combination with a polypropylene mesh with a titanized hydrophilic surface TiLoop Bra. In this case, a polypropylene mesh is fixed over the pectoralis major muscle and a smooth implant is covered with it. The first suture is formed at a distance of 1.5-2.0 cm from the submammary fold in the projection of the III rib, as shown in Fig. 2 and 5. Then, arcuately descending along the implant along the pectoralis major muscle, interrupted sutures are applied every 2 cm with an atraumatic thread, a smooth breast implant of the appropriate size and shape is placed in the formed pocket. The polypropylene mesh is wrapped with its free edge behind the lower pole of the breast implant and placed under and in front of the implant, preventing mesh wrinkling, the postoperative wound is drained and sutured.

EFFECT: method ensures preservation of the pectoralis major muscle function and aesthetic effect, reliable fixation, elimination of animated deformities of the reconstructed mammary gland, avoids damage to the pectoralis major muscle, reduces the time and volume of the operation, traumatism, and is technically simpler. In the postoperative period, moderate physiological lymphorrhea is noted, bed-days in the hospital are reduced, and the risk of capsular contracture is reduced.
