FIELD: medicine; oncology.
SUBSTANCE: invention relates to medicine, namely plastic, reconstructive surgery and oncology. After a skin-preserving mastectomy, the volume of the removed breast tissue is measured. To do this, the removed breast tissue is placed in a measuring cup filled with water, installed in a container with a larger diameter. Then the measuring cup is released, the volume of liquid is measured from a larger container in the measuring cup. Then, the size of the fitting implant is determined taking into account the volume of removed tissues obtained and by placing the implant in the postoperative bed, determining the symmetry of the result obtained. Then, for 12-28 days, 5 sessions of intravenous infusions are performed: a combination of pentoxifylline 100 mg/5 ml and Remaxol 400 ml - 1 time per day. After 12-28 days, when there is a twofold thickening of the cutaneous subcutaneous pocket, which is determined by ultrasound, the second stage of the operation is performed, selected a permanent implant is installed through the previous postoperative access to the skin pocket, then the wound is sutured in layers, with one drainage. From the next day, therapy is carried out with a combination of pentoxifylline 100 mg/5 ml and Remaxol 400 ml 1 time per day by intravenous infusion - 5-10 sessions.
EFFECT: method allows to significantly reduce the number of implant extrusions associated with tissue necrobiosis sites after skin-preserving mastectomy, due to wound healing, thickening of the subcutaneous pocket, improvement of microcirculation of skin flaps and nipple-areolar complex, increase the effectiveness of complex treatment, rehabilitation and reconstruction of the breast, cosmetic result of surgery and improve the psycho-emotional state and quality of life of patients.
