FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to ophthalmology, and can be used for treating severe forms of refractory glaucoma. Conjunctiva is incised and separated in the upper temporal region, the scleral region is exposed, the valve system is activated, the drainage body is anchored, the scleral vessels are coagulated, the scleral flap is separated. Tube is sheared, viscoelastic is introduced into anterior chamber. Drain tube is implanted into anterior chamber, scleral flap is attached to sclera, interrupted sutures are applied on conjunctiva. First, in the lower hemisphere of the eyeball free from previous operations, contact transscleral diode-laser cyclophobic coagulation is carried out in a microimpulsive mode in a cyclic manner in 4 mm from the limb, with power of 1000 mW, a duty cycle of 31.3%, a pulse duration of 0.5 ms, with period of 1.1 ms. Exposure for one cycle is 10 s, the first exposure cycle is performed along a circular arc of 90 degrees in one direction, the second cycle along the same arc of the circle in the opposite direction, subsequent cycles are similar to the first two, whole is 6 cycles. After valve system activation, distal end of drainage tube is filled with "Provisk" viscoelastic till its outlet from opposite side. Paracentesis is performed at 6 o'clock, then "Provisk" is introduced into an anterior eye chamber in amount of 0.2-0.3 ml and a drainage tube is implanted.

EFFECT: method enables preserving visual functions, reducing IOP by creating a new pathway of intraocular fluid outflow with an additional sparing effect on the area of the ciliary body.
