Abstract RU 2775129 C1

FIELD: medicine.

SUBSTANCE: invention relates to medicine, namely, to internal diseases and clinical immunology and allergology, and can be used to prevent the progression of autoimmune thyroiditis in the course of allergen-specific therapy. Allergen-specific therapy is conducted as follows. In patients with allergic rhinitis, based on the results of a clinical and laboratory examination including an evaluation of the allergological history, the nature of reaction of the patient's body to contact with the allergen in scarification allergic testing, as well as determination of the levels of thyroid hormones in the blood serum of the patient using the Siemens IMMULITE/2000 XPI automatic chemiluminescent immunoassay system with: polysensitisation to three allergens and response of the body of the patient to the administration of each thereof as a "strongly positive" – "+++", or "very strongly positive" – "++++" reaction, as well as levels of free T3 in blood serum in the range of 1.8 to 4.2 pg/ml, free T4 1.2 to 1.3 ng/dl, thyreotropin in the range of 0.4 to 4 mcIU/ml, level of antibodies against thyroglobulin no more than 40 IU/ml, and level of antibodies against thyroid peroxidase in blood serum no more than 35 IU/ml, a decision is made to conduct the first course of allergen-specific therapy using two or more etiologically significant allergens. For this purpose, increasing doses of allergens are injected subcutaneously into the shoulder area into the body of the patient in the allergological room using the injection method of allergen-specific therapy: the initial 0.1 ml dose of each allergen diluted in the ratio 1:10-7, at the stage of achieving the maximum therapeutic dose, allergen injections are made once every three days, at the stage of supporting therapy, a 0.5 to 1.0 ml dose of the allergen diluted in the ratio 1:10-1, allergen injections are made at an interval of two weeks. The condition of the patients is therein assessed by an allergy specialist before each subsequent injection and monitored for at least 30 minutes after injection. If an increased level of antibodies against thyroglobulin of more than 40 IU/ml, a level of antibodies against thyroid peroxidase of more than 35 IU/ml, and a level of thyreotropin of more than 4 IU/ml are detected in the blood serum of the patient in the course of allergen-specific therapy, the allergen-specific therapy is stopped, and treatment is used without a pathogenetic effect on the links of the allergic reaction. The clinical criterion for a favourable outcome of treatment is a reduction in the severity of clinical symptoms of the disease, reduction in the amount of requests for medical help in relation to allergic rhinitis from seven to three or less within one year.

EFFECT: method provides the possibility of preventing the progression of autoimmune thyroiditis in the course of allergen-specific therapy due to the proposed sequence of actions, preventing the development and progression of an immuno-mediated comorbid disease and leading to an improvement in the results of treatment of patients suffering from allergic rhinitis.
