FIELD: medicine.

SUBSTANCE: invention relates to medicine, namely to traumatology and orthopedics, and can be used when performing operative access to the structures of the shoulder joint. An incision of the skin and subcutaneous fat is performed in an oblique direction from the projection of the acromioclavicular joint to the proximal diaphysis of the humerus. Then a blunt linear dissection is carried out between the middle and anterior portions of the deltoid muscle and access to the upper and anterior parts of the shoulder joint is formed. Next, the skin and subcutaneous fat are shifted medially, the delta-pectoral interval and the cephalic vein are visualized, after the head vein is isolated, it is withdrawn upward along with the anterior portion of the deltoid muscle, and the pectoralis major muscle, medially and downward and access to the anterior and anterior parts of the shoulder joint is formed.

EFFECT: method provides excellent visibility of the surgical field in different vectors, as well as a reduction in injury due to the exclusion of cutting off the anterior portion of the deltoid muscle from the clavicle, which does not disrupt nutrition and innervation of muscle tissue, eliminates pronounced traumatism of surrounding tissues, reduces pain in the postoperative period and makes early activation possible.
