SUBSTANCE: invention relates to medicine, namely surgery. The skin is incised periareolarly with the transition to the skin of the border of the outer quadrants and subcutaneous separation of the gland. A periareolary incision is made, as shown in dwg. 1, with a continuation to the border of the outer quadrants with a length of 5 cm. The skin with subcutaneous fat is crossed, the superficial fascia is isolated, the skin is separated in all directions: the skin edge is lifted from the outside of the incision with hooks and the skin with subcutaneous tissue and superficial fascia is separated from the underlying breast parenchyma in stages along the perimeter with an electric knife. After complete separation of the parenchyma of the dome of the breast, the separation of the parenchyma of the gland adjacent to the chest wall is continued. A deep leaf of the superficial fascia is isolated with the underlying fascia of the pectoralis major muscle and the parenchyma is separated from it. Next, the parenchyma is removed in a single block and a silicone implant with a microtextured surface or polyurethane coating is installed in the wound cavity, after which the subcutaneous tissue and skin are sutured with an intradermal suture.

EFFECT: method makes it possible to eliminate the likelihood of breast cancer in patients with an increased risk of breast cancer due to the presence of genetic mutations BRCA1,2 and multiple sites of breast dysplasia, improve the aesthetic effect of subcutaneous mastectomy with endoprosthesis reconstruction, achieve a stable shape and symmetry of the mammary glands, reduce the number of postoperative complications, reduce the duration of anesthesia, blood loss and injury.
