The surgical field is processed, the stage-by-stage intersection of the skin and subcutaneous fat with a scalpel of the temporary feeding leg, the mobilization of the edges of the surgical wound, the application of a vacuum bandage. In this case, the first step is to dissect the skin with a scalpel, forming the boundaries of the skin-fat flap. Next, the subcutaneous fat tissue of the feeding leg of the flap is partially crossed to the depth necessary for subsequent modeling of the skin-fat flap. After that, the surgical wound is sewn up with sparce nodular sutures. The second step is the removal of the previously applied sutures. After processing the surgical field, the edges of the sutured wound are separated, the edges of the feeding leg are mobilized with a scalpel, crossing the subcutaneous fat to the depth necessary for subsequent modeling of the skin-fat flap. Sutures are not applied, and the edges are brought together and the surgical wound is drained by applying a vacuum bandage with a constant pressure of 90 mm Hg.

EFFECT: method makes it possible to form a skin-fat flap on a temporary feeding leg of the required thickness, create conditions for the prevention of ischemia of the skin-fat flap after cutting it out, shorten the operation time during the formation of a skin-fat flap, improve the results of surgical treatment of patients when performing flap surgery on a temporary feeding leg.
