FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to oncology and urology. An endoscopic instrument is advanced through the natural urinary tract and the tumor is removed with a single block "en block" by a laser unit in the "Ablation" mode with a wave energy of 1200 and 1800 mJ with a frequency of 10-12 Hz and removed by means of an extractor. In this case, a semi-rigid ureteroscope 9.5 Fr, 43 cm long, is passed retrograde through the natural urinary tract to the tumor, and under fluoroscopic control, an insurance metal conductor 0.035 gage is inserted through the working channel of the instrument to the pyelocaliceal system. The ureteroscope is removed and re-inserted past the insurance conductor. Further, a laser light guide with a diameter of 365 nm is brought directly to the tumor through the working channel of the instrument, and under passive irrigation conditions, tissues are dissected at the base in the “Ablation” laser installation mode with a wave energy of 1200 or 1800 mJ with a frequency of 10-12 Hz, and coagulation of bleeding vessels is performed. After that, the tumor is removed in a single block "en block" using an extractor. The operation is completed by placing an internal J-J stent 6/26 Fr along the insurance conductor and draining the bladder with a 16Fr Foley urethral catheter.
EFFECT: method allows avoiding nephroureterectomy and performing a hemodialysis program, obtaining better quality histological material, minimizing intraoperative blood loss, reducing operation time, increasing ablasticity, and thereby reducing hospital stay and treatment costs.
