METHOD FOR SELECTING THE TYPE OF EXTRA-INTRACRANIAL SHUNT BY THROUGHPUT CAPACITY IN ACUTE AND CHRONIC CEREBROVASCULAR PATHOLOGIES REQUIRING REVASCULARISING INTERVENTION Russian patent published in 2022 - IPC A61B5/02

Abstract RU 2784442 C1

FIELD: medicine.

SUBSTANCE: invention relates to the field of medicine, namely, to neurosurgery and vascular surgery. The following macro- and microhaemodynamics indicators are measured: minute blood volume QMBV, ml/min or cm3/min; volumetric blood flow rate in the common carotid artery (CCA) on the ipsilateral side of the perfusion deficiency zone QCCA, ml/min or cm3/min; volumetric blood flow rate in the sphenoidal segment of the middle cerebral artery on the ipsilateral side of the perfusion deficiency zone QM1, ml/min or cm3/min; volumetric blood flow rate in the middle cerebral artery territory on the contralateral side of the perfusion deficiency zone, ml/min/100 g or cm3/min/100 g. The resulting value is then recalculated by multiplying by a coefficient depending on sex and age, resulting in the value QCBF in ml/min/MCA or cm3/min/MCA. The required volumetric blood flow in the shunt Qs, ml/min or cm3/min, is then determined based on the original formulas. If Qs ≤ 10 ml/min or cm3/min, the shunt is not used. If 10 < Qs ≤ 50 ml/min or cm3/min, a low-flow shunt with a volumetric blood flow rate ≤ 50 ml/min or cm3/min is used. If 50 < Qs ≤ 100 ml/min or cm3/min, a medium-flow shunt with a corresponding volumetric blood flow rate is used. If Qs > 100 ml/min or cm3/min, a high-flow shunt with a volumetric blood flow rate > 100 ml/min or cm3/min is used.

EFFECT: possibility of determining the required volumetric blood flow rate (VBFR) in an extra-intracranial shunt (EICS) and selecting a shunt with such a throughput capacity that maintains the adequate values of the volumetric rate of cerebral blood flow (CBF) in the MCA territory where perfusion deficiency was initially determined.
