SUBSTANCE: invention relates to medicine, namely to traumatology and orthopedics, and can be used to perform operative access during interventions on the Achilles tendon. A skin flap is formed with the base facing medially from the tendon, while the beginning and end of the arcuate incision are located on the median line of the posterior surface of the lower leg, and the apex is on the lateral edge of the Achilles tendon. Then, a mirror-like fascial flap is formed with the base facing laterally from the Achilles tendon, while the beginning and end of the arcuate incision of the own fascia of the lower leg are located on the median line of the posterior surface of the lower leg, and the apex is on the medial edge of the Achilles tendon. Reconstruction of the Achilles tendon is performed. It is closed with a fascial flap, sutured to the edge of the own fascia of the lower leg, after which the proximal part of the wound is sutured, and the remaining skin defect is closed with a full-thickness skin autograft.

EFFECT: method provides the possibility of a wide view in the area of the reconstruction zone while maintaining the sliding apparatus of the Achilles tendon, reducing the frequency of marginal necrosis and excluding trauma to the sural nerve, as well as removing the postoperative scar zone from the area of constant load due to the access site and skin flap formation.
