METHOD FOR ELIMINATING THE DEFECT OF THE LOWER JAW AND SOFT TISSUES AFTER SURGICAL TREATMENT OF TUMORS Russian patent published in 2022 - IPC A61B17/00

SUBSTANCE: invention relates to medicine, namely maxillofacial, plastic and reconstructive surgery. The formation of the bone cavity is carried out with the resection of the lower jaw within healthy tissues, with a violation of its continuity. Subsequent reconstruction of an individually modeled titanium plate is performed. A flap is isolated for the subsequent covering of the reconstructive plate with it. In this case, the formation of the bone cavity is carried out by resection of the lower jaw with intraoral and external access, followed by its reconstruction with a titanium plate. Recipient vessels are isolated: a branch of the external carotid artery and two veins adjacent to it, and a free skin-fascial flap on the perforator vein of the medial gastrocnemius artery is used as a flap to hide the reconstructive plate. In this case, the vascular pedicle of the flap is represented by the artery of calf and the comitant veins, up to 16 cm long. The reconstructive plate is covered with a free skin-fascial flap. The vascular pedicle is passed without twisting, tension and bends, avoiding contact with the reconstructive plate to the area of the recipient vessels, anastomoses are applied between the medial calf and recipient arteries, medial calf veins and recipient veins, followed by suturing of wounds.

EFFECT: method makes it possible to minimize donor damage, namely, to reduce the traumatism of flap sampling, to provide visual control over the location of the vascular pedicle in the bone cavity, to avoid the second stage of treatment: cutting off the vascular pedicle and the need for corrective operations to thin the flap, to increase the functional and aesthetic effect of the operation in the donor area.
