FIELD: medicine.
SUBSTANCE: inventions group relates to medicine, namely to maxillofacial surgery and surgical dentistry, can be used for surgical treatment of patients with fractures of the condylar process of the lower jaw by osteosynthesis of fragments. An intraoral access is used along the anterior edge of the lower jaw branch with a transition to the anterior edge of the masseter muscle. The anterior edge of the lower jaw branch is reached, the outer surface of the lower jaw branch with visualization of the fracture line. In this case, the bone fragments are repositioned and held in the correct position. From the side of the skin in the parotid-masticatory region in the projection of the fracture, a puncture is performed with a trocar. The stylet is removed, a drill is inserted through the tube, which is used to form channels for fixing the screws. Titanium bone mini-plate is fixed with screws. The template is removed, the stability of the bone fragments and the bite condition are assessed. The tissues in the oral cavity are sutured with the introduction of glove drainage into the wound. An individual surgical template for osteosynthesis contains a body made of biocompatible plastic with recesses, made in the form of a plate 1.5 mm thick, repeating the relief with recesses for titanium bone mini-plates. It has three clasps: in the region of the lower jaw cut above the fracture line there is an upper clasp, on the posterior edge of the lower jaw branch below the fracture line there is a rear clasp. The upper and rear clasps pass to the inner surface of the lower jaw up to 5 mm. The anterior clasp reaches the anterior edge of the lower jaw branch without passing to the inner surface. In this case, the ratio of the width of the upper clasp and the rear clasp is 1:1, and the front clasp and rear clasp is 1:3.
EFFECT: method ensures the preservation of the integrity of the elements of the temporomandibular joint and the reduction of traumatism of the masticatory, lateral and medial pterygoid muscles due to the use of an individual template.
