METHOD FOR COMBINED TREATMENT OF SECONDARY UNCOMPENSATED GLAUCOMA Russian patent published in 2022 - IPC A61F9/07 A61F9/08

Abstract RU 2787152 C1

FIELD: medicine.

SUBSTANCE: invention relates to medicine, namely to ophthalmology, and can be used for combined treatment of secondary uncompensated glaucoma. For this, a conjunctival incision is performed paralimbally. A superficial rectangular scleral flap (SSL) is formed with dimensions of 5x5 mm per one third of the sclera thickness, with the base to the limbus. Further, 1-1.5 mm from the scleral spur with a trephine in the deep layers of the sclera, five trephine holes are formed in a checkerboard pattern with a diameter of 1.5 mm until the ciliary body is exposed, while three of the five holes are placed in one row parallel to the scleral spur at a distance of 1 mm from it at a distance of 0.2 mm from each other. The central of these holes is placed on the axis of symmetry passing through the center of the PSL, and the remaining two are placed in another row 0.5 mm lower than the previous row and form them, leaving the valve on the stem. The valve is folded inwards by 1/2 of its size between the sclera and the choroid to increase the uveoscleral outflow. Then the central hole, located on the axis of symmetry, is made through with a laser with a wavelength of 810 nm with a power of 0.3-0.5 W and a duration of 3-5 s until filtration from the posterior chamber of the eye appears.

EFFECT: invention provides prolongation of the hypotensive effect and stabilization of visual functions, creation of valves on the legs above two burr holes allows maintaining the surgically formed intrascleral cavity and creates an additional drainage outflow of intraocular fluid between the sclera and choroid.
