METHOD FOR DIAGNOSING IMPAIRED MOBILITY OF THE VOCAL FOLDS Russian patent published in 2023 - IPC A61B8/08

Abstract RU 2787835 C1

FIELD: medicine.

SUBSTANCE: invention relates to medicine, in particular to otorhinolaryngology, surgery, oncology, ultrasound diagnostics. An ultrasound examination is performed, in which the arytenoid cartilages are visualized, the parameters of the vocal folds are measured during inspiration from both sides. Additionally, the parameters of the vocal folds are measured at the moment of holding the breath. At the same time, during these functional tests on the echogram, the angles of deviation of each arytenoid cartilage from the line corresponding to the normal vocal fold closure line are measured. Then the difference between the angles of deviation during inhalation and breath holding from the respective sides and the length of the contraction of the vocal cords from the respective sides are determined. At the moment of holding the breath, the presence of the line of closing of the vocal folds and the contact of the vocal processes of the arytenoid cartilages are revealed and their shape is determined. When determining the difference between the angles of deviation of the arytenoid cartilages during inspiration and breath holding within 15-53° and the length of contraction of the vocal cords within 1.9-6.7 mm from the respective sides, as well as when identifying the vertical line of closing of the vocal folds and contact vocal processes of the arytenoid cartilages in the form of a "smile" the absence of a violation of the mobility of the vocal folds is diagnosed. When determining the difference between the angles of deviation of the arytenoid cartilages during inspiration and breath holding within 0-14° and the length of contraction of the vocal cords within 0-1.8 mm on one side, as well as when detecting deformation of the lines of closing of the vocal folds and contact of the vocal folds processes of arytenoid cartilages in the form of a "crooked smile" unilateral paresis of the larynx is diagnosed. When determining the difference between deviations during inhalation and breath holding 0-14° and the length of contraction of the vocal cords 0-1.8 mm on both sides, as well as in the absence of lines of closing of the vocal folds and contact of the vocal processes of the arytenoid cartilages, bilateral paresis of the larynx is diagnosed. When determining the difference between the angles of deviation during inhalation and breath holding from the respective sides within 15-53°, if the difference between the left and right sides is more than 15°, and the length of contraction of the vocal cords within 1.9-6.7 mm, deformities of the lines of closing of the vocal folds and contact of the vocal processes of the arytenoid cartilages in the form of a "crooked smile" laryngeal dyskinesia is diagnosed.

EFFECT: method makes it possible to detect paresis of the larynx in the early postoperative period, to carry out timely treatment, to increase the effectiveness of surgical intervention, to prescribe phonopedic exercises in a timely manner to restore the patient's voice function.
