FIELD: medical equipment.
SUBSTANCE: invention relates to medical equipment, namely to a method for adaptive control of infusion during vitrectomy. In the method, at the first stage, the intraocular pressure and the average ocular perfusion pressure are estimated, according to which the required pressure in the infusion line of the surgical system is set, which is recognized as corresponding to the personal target intraocular pressure and which is set in the infusion line taking into account the totality of the values of the average ocular perfusion pressure and intraocular pressure or taking into account each value separately. At the second stage, the intraoperative factor is assessed by the magnitude of the pulsation of the central retinal artery, on the basis of which the pressure in the infusion line is adjusted to ensure the target intraocular pressure. Correction of pressure in the infusion line is carried out in one of the pressure ranges, one of which is used in case of an existing risk of intraoperative retinal ischemia in the form of an increase in intraocular pressure and a decrease in the mean ocular perfusion pressure, the other of which is used in the absence of a risk of intraoperative retinal ischemia based on normal physiological values preoperative intraocular pressure and an indicator of the average ocular perfusion pressure, the third of which is used to increase the target intraocular pressure.
EFFECT: providing infusion pressure during vitrectomy, taking into account individual indicators of intraocular pressure and perfusion of the eye, which increases the safety and effectiveness of vitrectomy by achieving and maintaining the target intraocular pressure, reduces the risk of iatrogenic complications associated with impaired ophthalmotonus.
