FIELD: medicine; ophthalmology.
SUBSTANCE: method involves combined treatment of proliferative diabetic retinopathy complicated by central retinal rupture with accompanying retinoschisis and traction-rhegmatogenous retinal detachment. A posterior vitrectomy is performed, subretinal fluid is drained, a perfluororganic compound (PFOC) is introduced into a vitreal cavity, an internal boundary membrane (IBM) is stained and removed. PFOC is replaced with an air mixture, and thrombocyte-rich plasma (TRP) is applied to the rupture zone. After the posterior vitrectomy, the fibrovascular proliferations are removed, and all vitreoretinal tractions are relieved, and the subretinal fluid is passively drained in the retinal rupture area. Intraretinal fluid is partially drained from retinoschisis at the edge of rupture. Further, the vitreous body cavity is filled with liquid PFOC. After retinal edema and retinoschisis decrease in height, a second operation is performed, during which pre-stained IBM is partially removed around fracture. PFOC is replaced with air, the TRP rupture zone is covered, the vitreous body cavity is filled with silicone oil. Thereafter, transpupillary panretinal retinal laser coagulation (RLC) is performed on an outpatient basis using an automated coagulation navigation system. One month after completion of the course of transpupillary panretinal RLC, silicone oil is replaced with a balanced salt solution. In a particular case, the course of transpupillary panretinal RLC is performed in 3–4 stages. In a particular case, transpupillary panretinal RLC is performed by short-pulse exposure of 30–50 ms with a minimum amount of 1,600 coagulants. In a particular case, when performing transpupillary panretinal RLC, it is necessary to apply coagulates with an interval of not more than 0.25 of the coagulate diameter. In particular case, after application of TRP, RLC is performed in fracture zone.
EFFECT: method enables stabilizing the proliferative process on the retina.
