METHOD FOR ANESTHESIOLOGICAL PROVISION OF OPEN OPERATIONS ON THE FETUS FOR CORRECTION OF SPINA BIFIDA Russian patent published in 2023 - IPC A61B17/00 A61K31/02 A61K31/21 A61K33/06 A61K38/06 A61P43/00

Abstract RU 2789590 C1

FIELD: medicine.

SUBSTANCE: invention relates to medicine, in particular, to anesthesiology and resuscitation, and can be used for anesthetic support of open operations on the fetus to correct spina bifida. The method involves performing epidural anesthesia under aseptic conditions, a 25% solution of MgSO4, 16 ml, is injected intravenously into premedication for the purpose of tocolysis for 20 minutes, then the rate of administration of MgSO4, 25%, is reduced to 4 ml/h. Next, tracheal intubation is performed and combined enotracheal anesthesia is performed using an inhalation anesthetic, Sevoflurane, up to 1.5 minimum alveolar concentrations. Patients before uterotomy begin infusion of 1% Nitroglycerin solution at a rate of 0.2-0.3 mcg/kg/min. After performing surgical correction of the fetal spina bifida and suturing the uterotomy incision, the infusion of a 1% nitroglycerin solution, the administration of an inhalation anesthetic and a 25% MgSO4 solution of 4 ml/h is stopped. After the end of the surgical intervention, the patient is prescribed Atoziban at an initial dose of 6.75 mg, immediately after that, an infusion at a dose of 300 mcg/min is carried out for 3 hours, after that a prolonged infusion at a dose of 100 mcg/min is carried out for up to 45 hours.

EFFECT: use of the invention makes it possible to achieve stable intraoperative hemodynamics, high-quality postoperative analgesia, reduction of the need for opioid analgesics and early activation of the patient.
