SUBSTANCE: invention relates to medicine, namely to plastic and maxillofacial surgery. The pedicle of the velo-pharyngeal flap is cut off. Then, the cut-off velo-pharyngeal flap connecting the posterior pharyngeal wall with the soft palate is cut off from the attachment site on the posterior pharyngeal wall. Next, the velo-pharyngeal flap is dissected by a vertical incision into the anterior and posterior halves on the lateral surface with the transition to the tissues of the soft palate and the lateral wall of the pharynx, thus forming two muco-muscular layers: oral and nasal. Two muco-muscular layers are sutured between themselves and with the tissues of the lateral wall of the pharynx, thus forming a single muco-muscular layer and oral muco-muscular layer along the entire half of the palatopharyngeal ring. In this case, the lateral surfaces of the formed muco-muscular layers are sutured together. The wound surfaces of the formed two muco-muscular layers are sutured together, extending the distal ends to the borders of the transition of the lateral pharyngeal wall and the posterior pharyngeal wall.

EFFECT: method makes it possible to eliminate palatopharyngeal insufficiency after velopharyngeal plasty, reduce speech recovery time to 6 months and improve speech quality.
