FIELD: medicine; abdominal surgery.
SUBSTANCE: invention relates to surgery, and can be used when performing a longitudinal resection of the stomach. Through the transumbilical access, a 10 mm trocar is installed for the introduction of optics, 1 cm to the left of it, in another plane, a 5 mm trocar is installed for laparoscopic clamping. To the right of the optical trocar, a 5 mm trocar for the Harmonic ultrasonic scalpel is placed at a distance of 1 cm. The left lobe of the liver is sutured to the anterior abdominal wall with a furrier needle. Then, with the help of an ultrasonic scalpel, the stomach is mobilized along the greater curvature, after which the left 5 mm trocar is changed to a 12 mm trocar intended for the stapler. Traction of the stomach is carried out with a clamp through the right 5 mm trocar. A calibration probe is introduced into the stomach before cutting off with a stapler. A stapler is inserted through a 12 mm trocar and, taking into account the anatomy, the angle of its bend is adjusted to perform a full-fledged longitudinal resection. Peritonization of the hardware suture is carried out using a thread with notches. The stomach is removed from the abdominal cavity through the combined access between the right and central trocar, while the size of the access does not exceed the diameter of the resected stomach. The postoperative defect of the aponeurosis is sutured with interrupted sutures, and the skin is sutured with a cosmetic intradermal suture.
EFFECT: method allows to perform the operation using regular laparoscopic instruments, does not require the use of complex expensive equipment, due to the simultaneous performance of many simultaneous techniques on the organs of the abdominal cavity and small pelvis from one puncture, is accompanied by a less pronounced pain syndrome which reduces the rehabilitation time and also provides maximum cosmetic results.
