METHOD FOR SURGICAL MANAGEMENT OF MORBID OBESITY IN PATIENTS WITH TYPE 2 DIABETES MELLITUS Russian patent published in 2024 - IPC A61B17/94 A61B17/11 A61B17/72 A61B17/32 A61B17/34 A61M25/01

Abstract RU 2820732 C1

FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to abdominal and bariatric surgery, and can be used in surgical treatment of morbid obesity in patients with type 2 diabetes mellitus. Portion of the stomach is dissected above the antrum perpendicular to the lesser curvature of the stomach at a distance of one cassette of 60 mm. Stomach is vertically transected along the lesser curvature by directing the stapler towards the gastric fundus to form a wedge-like expanded small stomach and to create an antireflux valve by preserving the angle of His. Along the antimesenteric edge of the small intestine at distance of 2 m from the Treitz ligament, a hole is made with an ultrasonic scalpel, to which a small stomach is brought. Along the anterior wall of the lesser stomach above the line of the first stapling suture perpendicular to the lesser curvature, an opening is formed with an ultrasonic scalpel, through which the probe is brought out into the opening of the loop of the small intestine. A single-row continuous gastrojejunostomosis is formed with a notched suture on a calibration probe.

EFFECT: method is low-traumatic, provides a clear visualization of anatomical structures, a pronounced restrictive effect, reduces the risk of developing an inconsistency of the hardware suture and a fatal outcome, does not require the use of narcotic analgesics in the postoperative period, provides a stable remission of type 2 diabetes mellitus due to the formation of a small stomach of a larger volume and one anastomosis.
