FIELD: medicine; bariatric surgery.
SUBSTANCE: at the first stage, a fundoplication cuff is formed, the fundoplication is supplemented with a posterior cruroraphy. Further, in the second stage, with the help of a stapler, the stomach is crossed in the direction from the greater curvature to the lesser dividing the stomach into two parts, the small stomach is formed along the greater curvature, 15–18 cm long, on a calibration gastric tube with full preservation of the blood supply to the fundoplication cuff and the large curvature of the stomach. From the ligament of Treitz, 150 cm of the small intestine are measured, after which a T-shaped gastroenteroanastomosis is formed with a formed small stomach.
EFFECT: method makes it possible to stably reduce body weight with correction of metabolic syndrome, as well as to create an effective anti-reflux construction for the treatment of hiatal hernia and gastroesophageal reflux disease.
