FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to abdominal surgery, and can be used in surgical treatment of recurrent gastroesophageal reflux disease of hiatal hernia. Performing the previously installed fundoplication cuff with restoration of the primary anatomy of the oesophageal-gastric junction, performing the posterior and anterior cruroraphia along the diameter of the oesophagus, fixing the oesophagus to the right and left pedicles of the oesophageal opening and forming a new fundoplication cuff. Prior to formation of fundoplication cuff, degree of mobilization of posterior wall of gastric fundus is assessed, and with its sufficient mobilization, the posterior wall of the gastric fundus is brought under the oesophagus towards the lesser curvature, the anterior wall of the gastric fundus is brought above the oesophagus towards the lesser curvature, and both walls are fixed to the right wall of the oesophagus with at least two interrupted sutures, capturing in one suture successively the anterior wall of the gastric fundus, right wall of oesophagus at level of cardioesophageal junction and posterior wall of gastric fundus, with formation of symmetrical fundoplication cuff. With insufficient mobilization of the posterior wall of the gastric fundus, the anterior wall of the gastric fundus is drawn towards the lesser curvature and is fixed to the anterior wall of the oesophagus with at least two interrupted sutures, then the posterior wall of the gastric fundus is brought under the oesophagus towards the lesser curvature and fixed to the posterior wall of the oesophagus by at least two interrupted sutures.
EFFECT: invention provides reducing the risks of developing intraoperative and postoperative complications, as well as risks of developing recurrent disease by forming an adequate antireflux mechanism.
