SUBSTANCE: skin and subcutaneous fat are dissected, the mammary gland is isolated along the upper semicircle to the chest wall, and the mammary gland is separated along the upper semicircle from the pectoralis major muscle. At the same time, in the lateral corner of the surgical wound, the axillary fascia is dissected and the “sentinel” lymph node is identified. The node is sent for urgent cytological examination and the axillary fascia is sutured with absorbable suture material. Further, the subcutaneous tissue of the mammary gland continues to be isolated along the lower semicircle, and a mastectomy is performed. If tumor cells are found in the “sentinel” lymph node during cytological examination, axillary-subscapular lymphadenectomy is performed. In the absence of tumor cells, the operation is completed without performing lymphadenectomy.

EFFECT: method allows to reduce the operation time by removing the sentinel lymph node before the end of breast resection and mastectomy, to search for the sentinel lymph node using a portable gamma detector directly in the axillary tissue.
