FIELD: medicine; operative oncology.
SUBSTANCE: marking lines are applied, the projection flap is removed, and the skin is restored using a skin-fat flap of the anterior chest wall. A skin incision is made over the tumor node and a projection flap is removed. Next, the breast tissue with the tumor node is removed intraoperatively, and the sentinel lymph nodes are removed through a separate skin incision in the axillary region. Next, a skin-fat flap of the anterior chest wall is cut out in the form of a “tongue”. The upper border of the flap is the inframammary fold of the mammary gland, and the lower border of the flap is 4 cm below the inframammary fold. It is moved to the area of deficiency of the breast skin flap, an implant is placed in the formed musculocutaneous pocket, where the skin-fat flap of the anterior chest wall and the remaining breast skin is the upper surface of the pocket, and the pectoralis major muscle is the lower surface of the pocket. Next, drainage is installed in the area of the formed mammary gland and the subcutaneous tissue and skin in the area of the mammary gland, in the chest wall and axillary region are sutured layer by layer.
EFFECT: proposed method allows to preserve the pectoralis major muscle and preserve the functional component of the muscle, achieve symmetry of the mammary glands — operated and healthy, perform one-stage reconstruction of the mammary gland, remove a large amount of tissue with subsequent replacement with an implant, reduce the duration of anesthesia, blood loss and trauma, due to the absence the need for corrective surgery on the contralateral mammary gland.
