FIELD: surgery; coloproctology.

SUBSTANCE: at the beginning of the surgical intervention, the internal opening of the fistulous tract is visualized. The mucosa of the anal canal is dissected through this hole and a plug based on the biological material SIS is inserted. After that, the anal canal mucosa over the plug is sutured with separate interrupted sutures. A fistuloscope is inserted through the external opening of the fistulous tract, advanced in the direction of the internal opening to the external sphincter. Access is performed to the intersphincteric space, for which an incision of up to 1 cm is made along the intersphincteric groove in the projection of the fistulous tract. Under the control of a fistuloscope, a fistulous tract is isolated in the intersphincteric space, ligated and crossed distally to the applied ligature. A monopolar electrode is inserted through the fistuloscope. The fistuloscope is removed, the epithelial lining of the fistulous tract is coagulated, and the monopolar electrode is removed.

EFFECT: method is highly effective, it reduces the risk of recurrence of the disease, allows to reduce the size of the surgical wound when performing a surgical approach.
