FIELD: medicine; surgery; coloproctology.
SUBSTANCE: laser from BIOLITEC AG is used: Ceralas which operates at a constant wavelength of 1,470 nm and a power of 8 watts. The anus is divulged with a disposable rectal speculum. Internal hemorrhoids at 3, 7 and 11 hours are allocated. At the anoderm, a puncture is performed up to 0.2–0.3 mm, through which a metal conductor with a conical laser light guide from BIOLITEC AG is inserted: Ceralas in the shape of a pike. The latter is carried out through the tissues, bypassing the sphincter in the submucosal layer to the base of the visible internal hemorrhoid; then, in a continuous mode, with a wavelength of 1,470 nm, a power of 8 W, laser coagulation of the tissue of the hemorrhoid is performed, followed by displacement of the laser fiber within the pathological changes, with traction to the dentate line and subsequent removal of the laser fiber. After that, the vascular pedicle of the hemorrhoid is sutured and the prolapsing mucosa is further tightened with a continuous suture of 3 stitches to a depth of 2.0 cm.
EFFECT: method allows to optimize the mode of the energy directed to hemorrhoid tissue, to increase the efficiency and accessibility of the hemorrhoid treatment, to reduce the number of complications and severity thereof, to standardize the energy supply through the laser light guide, and to reduce the rehabilitation period after laser treatment.
