FIELD: medicine; ophthalmology.
SUBSTANCE: surgical treatment of Ahmed valvular drainage protrusion after antiglaucomatous surgery is performed. Conjunctival incisions on both sides of the erupted part of the drainage are performed. Identical incisions are made in the conjunctiva, starting from the erupted part of the drainage body, in opposite directions concentrically to the limbus forming a single incision. Next, the conjunctiva is separated from the Tenon capsule by 4 mm on both sides of the incision along its entire length. Then two incisions are made in the Tenon's capsule from the cut through part of the drainage body: the first one in the direction from the drainage body in the projection of the conjunctival incision and the second at an angle of 90 degrees from the first incision of the Tenon's capsule in the direction from the limbus. The length of each cut is 5 mm. The resulting triangular flap of Tenon's capsule is separated from the underlying sclera and pulled over the protrusion area above the drainage body, closing it, and fixed with interrupted sutures to the Tenon's capsule. The conjunctiva is displaced closer to the limbus, covering the sutures of the Tenon capsule with intact conjunctiva, and the conjunctival incision is sutured with a continuous suture.
EFFECT: method makes it possible to close the defect of the conjunctiva above the body of the drainage and, as a result, to prevent the development of an infectious process, normalize intraocular pressure and prolong the hypotensive effect after an antiglaucomatous operation.
