FIELD: medicine, surgery and oncology.

SUBSTANCE: invention relates to medicine, namely to endoscopic removal of the thyroid gland using high-frequency vibration in the range of 55,500 Hz. An antiseptic treatment of the surgical field and infiltration of subcutaneous fat in the chin area and in the submental triangle are carried out. A linear incision of the mucous membrane in the lower fornix of the vestibule of the oral cavity is performed, 1 cm long. Further, a 10-mm trocar with a valve for gas supply is placed in the center of the linear incision of the mucous membrane in the lower fornix of the vestibule of the oral cavity. On both sides of it, 5 mm trocars for endoscopic instruments are inserted through additional 5 mm incisions. Insufflation with carbon dioxide to a pressure level of 6 mm Hg is performed. To remove the thyroid preparation, a skin incision is made in the right axillary region 3.0 cm long. The subcutaneous fat is cut off to the medial pedicle of the right sternocleidomastoid muscle and a tunnel is formed. Through the formed tunnel, the endoscopic container with the medicinal product is first removed and then a drainage tube is installed in the bed of the removed thyroid gland, which is fixed to the Redon drainage.

EFFECT: method allows to provide adequate operational access to the thyroid gland and eliminate the appearance of a postoperative scar on the skin of the neck, avoid severe damage to the integumentary tissues of the chin area and ruptures of the mucous membrane in the lower fornix of the vestibule of the oral cavity, reduce the likelihood of damage to the mental nerve, reduce swelling and pain in the chin area, expand the indications for surgery and reduce the time of surgery.
