SUBSTANCE: invention relates to medicine, in particular to surgery. In the subclavian region, according to the markings, as in figure 1a, a skin incision 3.0 cm long is performed. The skin, subcutaneous fat and platysma are cut to the medial legs of the sternocleidomastoid muscles. A Farabef hook is installed to hold the musculocutaneous flap and create a cavity. Next, a 0.7 cm incision is made in the contralateral subclavian region, as in figure 1b, and an endoscopic instrument is passed through the trocar. The sternohyoid and sternothyroid muscles are separated with a harmonic scalpel. The second and third fascia of the neck are dissected and, focusing on the tracheal rings, the thyroid cartilage and the anterior tracheal rings are exposed. The pyramidal lobe of the thyroid gland, the upper and lower poles of the thyroid gland is reached. Then a hemithyroidectomy is performed.

EFFECT: method makes it possible to provide prompt access to the thyroid gland, exclude the appearance of a postoperative scar on the skin of the neck, extract a drug of any size from the wound, evaluate all groups of lymph nodes on the anterolateral surface of the neck and cope with all intraoperative complications without access conversion, avoid the risk of damage to large nerve trunks and vessels, use conventional devices for intraoperative hemostasis.
