SUBSTANCE: invention can be used in the treatment of fibrous adhesive changes in the subacromial space. The presence of fibrous-adhesive changes in the subacromial space is preliminarily detected, after which, on an outpatient basis, using ultrasound navigation, targeted needle fibrotomy of the identified fibrosis zone is performed. Scanning is carried out in the longitudinal direction along the tendon of the supraspinatus muscle in the area of its distal section and the fibrous-modified subacromial sac. Then the needle is injected lateral to the sensor along the long axis in the direction of the subacromial space and the needle is passed in the latero-medial direction at an angle to the plane of the sensor, while successively passing the subcutaneous tissue, the deltoid muscle in the direction of the subacromial space and layer-by-layer tissue infiltration is carried out in the place where the needle slides. Then, moving further, penetrate into the area of the fibrous-altered zone, under pressure, the therapeutic mixture is injected into this area, carrying out targeted hydrodestruction of the adhesion-fibrous zone until the sensation of the failure of the syringe plunger, the free distribution of the therapeutic solution and the appearance of a hypoechoic artifact on the monitor of the ultrasound machine.

EFFECT: method increases the efficiency of treatment of fibrous-adhesive changes in the subacromial space on an outpatient basis by performing an accurate and targeted effect on the local zone of fibrosis in the subacromial space in the form of needle fibrotomy, followed by hydrodestructive stratification of this zone.
