FIELD: medicine, urology.
SUBSTANCE: ureter is mobilized with its subsequent cutting off. The bladder in the region of the posterior-lateral surface is fixed to m.psoas from the side of the cut-off ureter. The bladder is dissected in the transverse direction, the edges of the wound are fixed with threads drawn transcutaneously. A submucosal antireflux tunnel is formed in the longitudinal direction, while the ratio of the length of the tunnel and the diameter of the reimplanted ureter should be at least 4:1. The distal ureter is introduced into the created tunnel and fixed to the bladder mucosa. A neostie is formed in a place close to the physiological one. An internal drainage stent is installed in the reimplanted ureter. In the place where the ureter enters the antireflux tunnel, it is fixed.
EFFECT: method reduces the trauma of surgery, the frequency of postoperative complications in the long term, and also provides faster activation of the patient; provides an opportunity to perform reimplantation of the ureter in difficult clinical cases, when there is a pronounced deficiency of length and there is no possibility of using standard methods of surgical intervention, allows to perform the entire range of transureteral endoscopic interventions on the upper urinary tract if there are medical indications in the separated postoperative period, including for the treatment of urolithiasis illness.
