FIELD: traumatology and orthopedics.
SUBSTANCE: If irreparable damage is confirmed, centralization of the proximal tendon of the long head of the biceps is performed. Then, the tendon of the long peroneal muscle is taken, followed by stitching only the distal part. Next, the distal part of the lower portion of the trapezius muscle is isolated and cut off from the spine of the scapula, then the cut off part is wrapped with the proximal part of the autograft of the long peroneal muscle and stitched. An additional arthroscopic port is formed medially to the posterolateral port, then a through channel is formed using a pin in the posterolateral section of the greater tubercle of the humerus towards the intertubercular groove. Next, the channel is reamed along the needle to a diameter corresponding to the diameter of the graft. With the help of a conductor thread, threads of a button fastener are passed through the formed channel. The free part of the tendon of the long peroneal muscle is passed through the loop of the button fastener, forming a duplication. The button is fixed on the opposite cortical layer and the graft is immersed in the formed channel. Then the lower portion of the trapezius muscle is sutured with a fixed second end of the graft, and after maximum tension, the ends of the threads are tied together.
EFFECT: reducing the risk of recurrence of ruptures of the rotator cuff of the shoulder, postoperative infectious complications and facilitating postoperative rehabilitation due to the combination of techniques of the claimed invention.
