NON-MEDUCATIONAL METHOD OF TREATMENT OF ADYNAMIC DEPRESSIONS Russian patent published in 2023 - IPC A61N2/00

Abstract RU 2801161 C1

FIELD: medicine, psychiatry.

SUBSTANCE: invention can be used to treat depression with a predominance of the adynamic component. Transcranial magnetic stimulation with intermittent theta bursts (iTMS) is performed in combination with virtual reality technologies with multimodal sensory impact (visual, auditory, tactile) (VR). The left dorsomedial prefrontal cortex (lPFC) is modulated with a MagPro R30 magnetic stimulator (Magventure, Denmark). Stimulation parameters are determined by the biphasic Burst (Theta Burst) stimulation protocol, in which 1 stimulus consists of 3 impulses, the interval between which is 20 ms (i.e. frequency 50 Hz), a burst of 10 such stimuli with a frequency of 5 Hz is repeated 20 times with an interval of 8 seconds. The total number of theta stimuli is 200. Further, to enhance neuromodulation, an impact is immediately carried out using virtual reality technologies on the Reviver neurotrainer (Russia). iTMS and VR procedures are carried out daily with a weekend break, with the duration of each session being 4 and 20 minutes, respectively, followed by an assessment of undesirable effects and control over the patient's water-intake regime. The general course is 20–30 procedures.

EFFECT: method provides highly effective therapy by activating the mechanisms of neuronal plasticity and restoring functional connectivity in the brain with complex multimodal activity-dependent stimulation, which reduces ideomotor inhibition, thymopathy in adynamic depressions of various structures, reduces the duration of treatment, overcomes therapeutic resistance, increases the rehabilitation potential and quality of life of the patients.
