METHOD OF REMOVING OROSTOMAS IN THE REGION OF THE FLOOR OF THE MOUTH Russian patent published in 2023 - IPC A61B17/24 

FIELD: medicine, plastic surgery and maxillofacial surgery.

SUBSTANCE: incision is made in the submandibular region on the side of the osteonecrotic process or defect. The tissues are dissected in layers with excision of the orostomy to the edge of the lower jaw. Resection of the jaw or bone edge defect is performed. A tongue-shaped flap is cut out on the pedicle, the base of which is located in the projection of the sternocleidomastoid muscle at an angle of 120° to the sternocleidomastoid muscle. A fascial fat flap is formed with a width equal to the length of the orostomy, and a length of 1 to 5. The flap is dissected, it is deployed in the direction of the mucosal defect, the contact with the underlying tissues is maintained. The flap is sutured with "U"-shaped sutures, retreating 2 mm down from the inner edge of the mucous membrane. The edges of the mucosal defect are preliminarily excised. The edges of the defect are sutured with non-absorbable suture material.

EFFECT: method allows to close a through defect of the floor of the oral cavity by restoring the integrity of the mucous membrane of the sublingual region, replenishing the volume of the tissues of the floor of the oral cavity, eliminating excessive tension in the tissues, ensuring mobility and good blood supply to the displaced flap.
