FIELD: medicine; anesthesiology-resuscitation; oncology; otorhinolaryngology; rhinology and rhinosurgery.
SUBSTANCE: in endoscopic oncosurgery of the nasopharynx in children, the method of combined anesthesia with a regional component is used. The method includes endotracheal anesthesia, bilateral pterygopalatine anesthesia with palatine access and infraorbital anesthesia on both sides. When conducting regional anesthesia with the spread of the tumor to more than half of the hard palate, bilateral infraorbital anesthesia is performed. After completion of the surgical intervention, before extubation of the trachea, using a nasal tampon from a compressed microporous sponge with hydroxycellulose, infiltration anesthesia of the nose is performed from three points according to Weisblat.
EFFECT: method allows to create a guaranteed perioperative analgesia due to the regional component, reduce the risk of trigeminocardial reflex manifestation, provide comfortable conditions for the surgeon's work: minimization of bleeding and reduction of the body's stress response to surgical trauma, allows prolonging the analgesic effect after surgery up to 8–10 hours, and also makes it possible to speed up the treatment process, contributing to early rehabilitation after surgery, and to minimize the use of narcotic drugs.
