FIELD: medicine; vascular surgery; X-ray endovascular surgery.
SUBSTANCE: ultrasonic duplex scanning of a pulsating hematoma is performed. The size of the pulsating hematoma and the presence of additional cavities are estimated by the largest diameter, the neck is located with an assessment of its length and width by the largest diameter. In the color and wave Doppler mode, the nature of the blood flow in its additional chambers and neck, as well as in the feeding artery and its distal branches, is assessed. Then, after treating the area of the pulsating hematoma with an antiseptic solution, in the projection of the middle of the cavity of the pulsating hematoma, a percutaneous puncture is performed with either 21G or 18G needle without using local anesthesia. Further, under ultrasound control in the B-mode, the location of the needle tip is determined, which is then placed above the middle of the upper wall of the pulsating hematoma, after which the cavity is punctured. Further, after obtaining a needle of pulsating blood flow from the cannula, a nitinol 0.018 conductor without a hydrophilic coating with a flexible tip is inserted into it, the direction of the needle is chosen not coinciding with the direction of the channel of the neck of the pulsating hematoma. The conductor is inserted into the cavity of the pulsating hematoma until resistance appears. Under ultrasound control, the conductor is folded in the cavity of the pulsating hematoma, after which the needle is removed, and the conductor is left in the cavity under constant ultrasound supervision until complete thrombosis of the main cavity of the pulsating hematoma and its additional chambers is reached, then the conductor is removed.
EFFECT: method does not require anesthesia and the abolition of anticoagulant therapy, additional compression with a sensor is not necessary, while, as a rule, cavity thrombosis occurs in much less time than with other methods, the method is accessible, safe, simple and fast, cost-effective, there is no need to use additional instruments, contrast agents and ionizing radiation, the method does not require the conditions of an X-ray operating room and can be performed in the ward or dressing room.
