METHOD OF REMOVING OROANTHRAL FASTENING WITH THE HELP OF FAT BODY OF THE CHEEK BY THE TUNNEL METHOD Russian patent published in 2023 - IPC A61B17/24 

FIELD: medicine; surgical dentistry; maxillofacial surgery.

SUBSTANCE: horizontal incision of the mucous membrane is made in the region of the transitional fold at the level of the crown of the second molar, 1.5 cm long. A full-thickness subperiosteal tunnel is created from the vestibular edge of the socket of the extracted tooth or the defect formed after the removal of the failed dental implant, to the incision of the mucous membrane of the transitional fold. The buccal spur of the fat body of the cheek (FBC) is mobilized over the mucosa of the alveolar process to the area of the oroantral anastomosis. The end part of FBC is captured with suture material passed through the tunnel. The lumen of the oroantral anastomosis is closed with a buccal spur of FBC passed through the tunnel. FBC is fixed to the mucosa of the alveolar process in the area of the oroanthral anastomosis with interrupted sutures. The incision is closed with interrupted sutures.

EFFECT: method allows to eliminate the oroanthral anastomosis with the maxillary sinus, reduce the risk of decreasing the depth of the vestibule of the oral cavity and infection of the postoperative wound, reduce the likelihood of fragments of the FBC and their subsequent necrosis, and reduce the volume of subsequent rehabilitation measures associated with the restoration of the dentition.
